Getting ///////// output with this model across all quants

Hey,
I've been struggling with this issue for awhile now. Basically, ill be running a session in opencode and everything seems to be going fine, but at random times it just starts outputing //////////// indefinitely. I have to eventually force quit the model. Any ideas on why this might be happening?
I've also been having CUDA errors. They might be related but im not sure.
