Add model card for M-GDM

This PR adds a comprehensive model card for M-GDM (Metadata-Guided Diffusion Model), which was presented at CVPR 2025.
The model card includes:
- Metadata for
library_name(diffusers) andpipeline_tag(image-to-image). - Links to the paper and official GitHub repository.
- A summary of the two-stage restoration method.
- Basic usage and evaluation instructions from the original repository.
- Citation information.
